Spontaneous cervical cephalic arterial dissection and its residuum: angiographic spectrum.

Cervical cephalic dissections are uncommon acute disruptions of the arterial wall occurring predominantly in middle-aged women. Clinically, most patients present with unilateral headache, oculosympathetic palsy, or ischemic neurologic symptoms. Usually, a single internal carotid artery, predominantly the right, is affected, but simultaneous multivessel dissections are evident in about one-third...

Cervical arterial dissection.

Background and Purpose—Cervical arterial dissection is a major cause of stroke in young adults, yet despite standard treatment with anticoagulants or antiplatelet drugs, its management remains uncertain. The goal of this study was to assess the natural history of the disorder and to decide on the feasibility of a therapeutic trial. Methods—Collaborating members of the Canadian Stroke Consortium...

Spontaneous intracranial carotid artery dissection.

A previously healthy 46-year-old woman had the abrupt onset of aphasia and right hemiplegia from a large left-hemisphere infarct. At postmortem examination the cause of the infarct was dissection and occlusion of the intracranial carotid artery. No preexisting abnormality was found to account for the dissection. This uncommon cause of stroke and its pathogenesis are discussed.

Management of spontaneous dissection of the cervical carotid artery.

This manuscript reviews the management of patients with spontaneous dissection of the cervical internal carotid artery (sICAD). Recommendations are not based on controlled-randomized trials, but on case-control and observational, hospital-based studies, and case reports. Vascular risk factors seem to be as prevalent in patients with sICAD as in age-matched, healthy volunteers. Stroke prevention...

Interventional Management of Intracranial Stenosis

Worldwide, intracranial atherosclerosis is the most common cause of ischemic stroke and is associated with a high risk of recurrence. Endovascular therapies including angioplasty and stent implantation may help in secondary stroke prevention due to intracranial stenosis, however rigorous appraisal of clinical efficacy is currently lacking. This review aims to introduce the basic concepts involv...
